Job Summary:
Provide case management to City Union Mission guests working towards their most viably-secure level of independence through goal-setting, mentoring, accountability, instructions and coordination of services.
Duties/Responsibilities:
● Administer individual intake and assessments to identify goals and objectives needed for guests to achieve outcomes towards their most viably-secure level of independence.
● Assist WFC guests in the completion of necessary applications and/or forms.
● Assist in creating an individualized goal-based comprehensive case management plan, including services in the areas of budgeting, referrals to resources, life skills, medical assistance, legal advocacy and time management.
● Meet regularly with guests to monitor and evaluate progress on case management goals and outcomes, provide referrals and advocacy to other services, and develop corrective action revisions to the plan as needed.
● Maintain accurate, updated and organized participant files, including receipts, documents and case notes, and ensure file privacy and security.
● Instruct and supervise guests in adherence to Center policies and requirements, provide informed support for safe exits, lead effective conflict resolution and crisis interventions, and coordinate with appropriate resources for aftercare and follow-up.
● Serve as a positive contributing member of the program team, and be responsible to communicate with appropriate staff, teach classes where needed, supply reports, attend meetings, etc. to ensure unity with the purpose and goals of the Mission.
● Seek to exemplify Jesus Christ daily and be prepared to share the gospel as opportunities arise.
● Perform other duties as required.
City Union Mission Requirements:
● Be a born again, mature Christian able to clearly communicate the Gospel as opportunities arise during the workday. (1 Peter 2:21, 3:15)
● Have a desire to serve the Lord through widely varied responsibilities to advance ministry to those currently homeless, and the urban poor of Kansas City, with a compassionate heart.
● Agree to City Union Mission’s Statement of Ministry:
o Be a genuine believer in the Lord Jesus Christ, as expressed by personal testimony and conduct.
o Be a member in good standing of a recognized protestant, evangelical church for one year or more.
o Have a genuine concern for the ministry of City Union Mission.
o Have a minimum of one year of sobriety from alcohol and drugs and six months from tobacco.
o Agree to abstain from alcoholic beverages, illegal drugs, and tobacco while employed by City Union Mission.
o Have a lifestyle that is free of sexual sin as clearly defined in the Bible.
Required Skills/Abilities:
● Demonstrate knowledge in case management assessment, planning, implementation and coordination of services.
● Demonstrate experience exhibiting good judgment, sensitivity, neatness, accuracy, and dependability.
● Must be able to demonstrate effective and clear communication, and be flexible and adaptable to work in a fast paced environment.
● Demonstrate computer literacy in Google and Microsoft applications.
Education and Experience:
● Required: Associate’s degree and 3 years of related experience
● Preferred: Bachelor’s degree in social services or a related field
Physical Requirements:
● Prolonged periods of sitting/standing/kneeling, etc.
● Ability to ascend/descend stairs multiple times throughout the shift.
● Must be able to lift 40 pounds at times.
● Must be able to operate motor vehicles, such as 15-passenger vans.
Travel Requirements:
● Ability to drive to our campuses in Kansas City and in Warsaw or to local service agencies.
